First of all...beef today is much leaner unless you go to a good butcher. AS for fat content....you will render off all the unnecessary fat when you cook any meat. If you roast, braise or grill meat your fat content will be much less than if you fry any meat product.

Before cooking, kind of like a quarter pound of beef is before its cooked and less after. Some people add stuff to make it fattier, but if you dont add anything it shouldnt change.

to take out even more calories and fat rinse the meat under hot running water before finishing the dish
it depends on how you cook it and how much fat you drain off; could be more, could be less
(it does give you a good idea of the content though)
rinsing under water takes flavour away so I would not recommend doing that
