You can do something really speacial like a suprise party or you can give her nothing beacause love is all it counts.
I think that it is a mistake to suprise her. My husband's family did that for his mother's 80th birthday and she fainted. The shock could be too much for an elderly person. Just have a regular party and let her invite anyone she wants.

In fact, the family can compile some old photos of her and made into postcards to be given away to guests. or made t-shirts of the yonder days to be given to guests. It depend also on the budget for the party--to splurge or not !!!
Get the Mayor of the town where she grew up, to make it her day, with a proclamation on paper. It will be a nice addition to everything else, and make you feel good too.
I have an idea why not try to find out if there are any friends that she has lost contact with and if you can find them in time invite them to the party.
