People can get fat because of many different reasons, ie. overeating, hormonal imbalance, lack of exercise, medications, stress, etc.
Ugly is an opinion. There are plenty of thin people with ugly personalities.

Because they don't take care of their bodies. They eat fast food, take prescription drugs, don't exercise, work too much that causes stress and don't socialize outside of work. Over the years of badly treating it.they become overweight, get illness, and look sick.

Ugliness I don't have an answer for other than an unfortunate heredity.
Fat people though, it can either be laziness/overeating, medical problem, or eating disorder.
Not everyone can help it, some people are put on medications that make them gain weight, some people have health conditions that do the same, other people have BED ( binge eating disorder) which is an actual disorder, not to be confused with casual overeating.
And then of course there are those people who don't give a **** and overeat and lay around all day.
But please don't judge all fat people calling them lazy pigs, some might deserve it, but not all of them.

People aren't born fat and ugly everyone is born a beautiful child, the way god made them. But sadly as they got older they were either told they were fat or ugly and soon if they were told it enough times they actually started to believe it but the truth is they really aren't fat and ugly its just since they believe it everyone around them will start believing it too and that's one reason. Another reason simply is because some people just don't care. Some people think that if they don't take care of themselves, don't eat right, and ruin their bodies that nothing will happen. HELLO!!! NEWSFLASH !!! Something is going to happen you will become FAT & UGLY !!

Well, speaking as someone who is fat and, I suppose, ugly, I will tell you.
I have hypothyroidism. In other words, a very slow metabolism. I take medication for it, but the weight is coming off very slowly. I was also on a medication that caused me to gain significant weight. Because of the thyroid thing, the weight is still with me.
I do care about my appearance, I exercise regularly ( I walk 3 or 4 miles every day), and I eat a healthy diet - lots of fruit & vegetables, low fat, low carb.
So - am I lazy? No. Am I unfortunate? Perhaps. Am I trying? Yes.
why do people get anorexic and ugly
